The nurse provides breastfeeding classes, health education, risk assessments, mental health services, oral health services, and care coordination. Clients include pregnant and postpartum women, children, adolescents, and parents or guardians. Client services are recorded accurately and submitted to the supervisor through required reporting systems.
Community outreach includes identifying gaps in services, planning activities, establishing priorities, coordinating logistics, and evaluating results. Health promotion programs are adapted to community needs, participant feedback, and program goals. The nurse also identifies opportunities to expand participation in maternal, child, and adolescent health programs.
Community Outreach And ReferralsRelationships with community organizations, service providers, agencies, and stakeholders are maintained through meetings, committees, outreach, and liaison work. The nurse identifies available resources and keeps a working directory current so clients can receive appropriate referrals. Information about emerging services is shared with team members.
Qualifications And Professional PracticeAn associate degree in nursing, Iowa Registered Nurse licensure, CPR certification, and one year of related work experience are required. The nurse must communicate orally and in writing, organize community activities, prioritize deadlines, maintain confidentiality, follow safety protocols, and serve the public professionally.
