We are recruiting a Prescribing Technician to join our successful,innovative and happy practice. The post holder will be primarily responsible for the delivery of a patient-centred medication management service.

Main duties of the job

Ensure the provision of high quality, customer focused services to support the practice and meet service demands.

Ensure healthand safety requirements are met and that the Practice complies with the Health& safety at Work legislation, carrying out a systematic program of risk assessments

Ensure infection control policies are adhered to

Manage data collection to support data requirements and claims

About us

The current list size of Danetre Medical Practice is approximately
12,800. Any increase over the last ten years has been due mainlyto; natural change (more births than deaths); building development within

Daventry itself and more recently migration from the rest of the UK and overseas. The boundaries of the Practice show we are large in terms of geographical size, with the majority of the land classified as rural.

The practice itself is located in close proximity to Daventry Town centre in a modern building adjoining Danetre Hospital (a community hospital which opened in September 2006) offering a wide range of services and delivering a high standard of care. Also attached to the Practice operating independently is Boots Chemist.

Currently the practice works with 5 x GP partners, 1 x Managing Partner, 6 x salaried GPs, 3 xPractitioners, 3 x Clinical Pharmacists and over 250

Hrs of Clinical nursing support.

Job responsibilities

Towork as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.

Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, thosewith multiple long term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme).

Providespecialist expertise in the use of medicines while helping to address boththe public health and social care needs of patients and help in tackling inequalities.

Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, while contributing to the quality and outcomes framework and enhanced services.

Through structured medication reviews support patients to take their medications toget the best from them, reduce waste and promote self-care.

Havea leadership role in supporting further integration of general practice withthe wider healthcare teams (including community and hospital pharmacy) tohelp improve patient outcomes, ensure better access to healthcare and helpmanage general practice workload.

ADDITIONAL RESPONSIBILITIES

Develop relationships and work closely with other pharmacists across networks and thewider health system.

Takea central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) and anticoagulation.

Communicate effectively with patients and carers,recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Adhere to quality governance systems, processes across the Practice and its activities; managing data collection to support data requirements/claims; ensuring information governance standards are implemented and maintained

Ensure health and safety requirements are met

Attend training courses identified as essential or beneficial to the role and take responsibility for identifying own learning needs

Ensure all significant events are reported and discussed as appropriate and facilitate a full understanding of clinical governance requirements within the practice

Person Specification Qualifications
  • You will be a Qualified Pharmacy Technician with NVQ 3 or BTech in Pharmaceutical Science and registered with the GPhC.
  • Applicants with the above qualifications who lack experience will be considered
  • Computer literate with MS office processing skills
  • Knowledge of changing NHS and Social Care responsibilities
  • Awareness of diversity and equality and ability to deal with personal details in a confidential manner
  • Excellent communication (written and oral) and interpersonal skills with the ability to work independently and as part of a multidisciplinary team
  • Previous experience of working in Social Care in medication management, but not essential
  • Pro-active and self-motivating
