Yolo Cares is seeking a Registered Nurse (RN) for the Night Shift to provide after-hours nursing assessments, triage, and in-person care to hospice patients and families in Yolo and surrounding counties. You will report to the Clinical Manager and participate in coordinated interdisciplinary care to ensure comfort and symptom management.
Responsibilities include telephone triage, home visits when needed, and documentation in the EMR, while collaborating with physicians, nurses, and social workers
