Part-time Performer Dentist required for 2-4 days in a MODERN, PREDOMINANTLY PRIVATE & INDEPENDENTLY OWNED practice in Dawlish, Devon

• Small UDA contract (negotiable)

• Practice is 75% PRIVATE
• Flexible with days on offer
• UDA rate £13.50 (2500 available - Flexible)
• £500 per day for NHS Access contract (1 day every other week only)
• Visa Sponsorship licence
• Hours 9.00am - 5.15pm
• Existing patient list plus new patients
• 45% - 50% Private
• 50% split on Lab bills
• 4 Modern, air conditioned, fully computerised surgeries
• Digital x-rays, SOE
• Rotary Endo
• Full support team including a Dental Hygienist/Therapist & Treatment Co-Ordinator
• On-site parking available

• Full NHS Performer Number
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
