Senior Mental Health Practitioner; MH/LD Nurse
We are seeking a dynamic, highly motivated and enthusiastic Mental Health or Learning Disability Nurse for a Senior Mental Health Practitioner (MHP) post with the Forensic Intellectual and Neurodevelopmental Disorder team (FIND) based at Easby House.
The FIND Team is managed within the Secure Services directorate in Devon Partnership NHS Trust and is part of the Community Forensic Services (CFS). Since 2021 the CFS has integrated the management of the SCFT, FIND and FPD teams to ensure a single point of entry to the service, centralising triage and gate‑keeping functions.
FIND aims to reduce length of stay for secure care patients with a primary diagnosis of Learning Disability, Autism Spectrum Condition or Acquired Brain Injury and to reduce the use of out‑of‑area or specialist placements, supporting the use of South West network beds. This involves a structured approach of direct work with patients (intervention, specialist assessment, case management) and indirect work with staff teams (consultation, training, supervision).

The postholder will be a core member of a multi‑disciplinary FIND service and will be responsible for carrying out the functions of that team, including managing a case load of varying complexity with a principal focus on service users who would benefit from specialist forensic LD and A intervention. The postholder will provide expert assessment and treatment for people with complex difficulties and their carers, including recovery care coordination (design and coordination of personalised treatment packages that are evaluated).
The postholder will deliver care management, assessment, care planning, review and evaluation. Liaison with a wide range of statutory and non‑statutory agencies is central to this role. The postholder will contribute to the clinical leadership of the team, providing supervision for other members and participating in team and service development and evaluation.
Working as part of Devon Partnership NHS Trust, the postholder is expected to work under pressure and without direct supervision, prioritise tasks, manage workload, and support colleagues where required.

Knowledge and Skills- Understanding of the Mental Health Act and Mental Health Capacity Act requirements
