About The Service

Stewart Gardens is part of our Devon Supported Housing service, providing housing‑related support to people leaving hospital or residential settings who have low to medium support needs. Using a recovery‑focused approach, we empower people to build confidence, develop independence, and achieve their personal goals.

What you’ll be doing Role

As a Mental Health Support Worker, you’ll work closely with the people we support, helping them take positive steps towards greater independence and wellbeing. Your role will include:

  • Providing person-centred support tailored to individual goals and recovery plans.
  • Building positive, professional relationships based on trust, respect, and encouragement.
  • Supporting people to access local services, community opportunities, education, employment and social activities.
  • Undertaking needs assessments, support planning and risk management activities.
  • Working collaboratively with healthcare professionals, external agencies and community partners.
  • Maintaining accurate records and contributing to service quality and outcomes monitoring.
  • Promoting wellbeing, independence and self‑management skills.
  • Supporting with personal care where required, including assistance with washing, dressing, toileting, eating and drinking.
