Position: SUBSTANCE USE NAVIGATOR - ED

SUBSTANCE USE NAVIGATOR - Emergency Department

Miami Valley Hospital, 1 Wyoming St, Dayton, OH 45409
Night shift / Full‑time: 10:00 p. m. – 8:30 a. m.
4 days per week + every other weekend.

Overview

The Substance Use Navigator coordinates and supports patients in the Premier Health Emergency Department/OB triage and inpatient medical floors who have been identified with a substance use disorder (SUD). The role includes performing SUD assessments, linking patients to Medications for Addiction Treatment (MAT) and community resources, educating patients, communicating with clinical and administrative staff, monitoring outcomes, and participating in grant‑related activities.

Duties & Functions
  • Participate in screening for patients with SUD/OUD.
  • Establish positive relationships with patients, treatment entities, community resources, and peer recovery support services.
  • Coordinate follow‑up for patients in ED, OB triage, and on the medical floor with SUD/OUD.
  • Advocate and act as liaison between patients and emergency/inpatient medical staff.
  • Assist patients in eliminating barriers to accessing MAT in the ED/OB triage and inpatient floors.
  • Coordinate warm handoff to appropriate treatment entities (outpatient, inpatient, detox, or residential).
  • Develop and maintain a documentation system for accurate follow‑up and program reporting.
  • Enter encounter data into EPIC and other data‑collection systems.
  • Participate in regular meetings, education, and training relevant to the role.
  • Perform other administrative duties as assigned.
Minimum Education

Associate degree (Preferred: Allied Health, Chemical Dependency Concentration, or related health services field).

Licensure/Certification

Chemical Dependency Counselor Assistant (CDCA) or obtain within the first 6 months of employment.

Experience

1–3 years of job‑related experience. Prior roles as Community Health Worker, Peer Recovery Supporter, Case Manager, or emergency department staff are preferred. Experience with medical terminology, substance abuse case management, grants, and MS Office is a plus.

Other Requirements
  • Knowledge of MAT and SUD treatment pathways.
  • Strong communication and collaboration skills.
  • Ability to work night shift and on-call weekends.
