Job DetailsThe Medication History Pharmacy Technician, with supervision of a pharmacist, assists in the process of obtaining medication history for patients who will be admitted to the hospital. The focus of this technician is the accurate collection of prescription information, allergy information, patient and/or family/guardian interview, and documentation of those activities. The complete collection of medication information may also include contacting pharmacies, long-term care facilities, and prescribers.
Additionally, the Medication History Pharmacy Technician works under the supervision of the pharmacist following established policy and procedures set forth by the hospital and pharmacy department. This role meets the requirements of being a qualified pharmacy technician as established by the State Board of Pharmacy and holds a Certified Pharmacy Technician license from a State Board of Pharmacy accredited agency. The technician adheres to safety practices to ensure quality care for patients and maintains competencies outlined in pharmacy department policies.
The technician shows familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices.
Hours: 8-days on/6-days off- evening shift-
Education & Certification- Education: HSD/GED is required
- Certification/Licensure: CPhT - Certified Pharmacy Technician - State of Ohio Board of Pharmacy is required
