Position: COMPLETION TECH

Position: Chart Completion Tech

Department: Health Info Management Services

Facility: Miami Valley Hospital

Responsibilities
  • Under general supervisor, prep, scan, and validate all types of paper documents into the optical scanning system to achieve chart completion.
  • Scan and validate all loose reports that are sent to the department.
  • Analyze medical records for deficiencies in chart documentation.
  • Release records for continuity of care requests, ensuring HIPAA guidelines are followed to reduce potential liability.
  • Monitor incomplete charts and follow up with physicians.
  • Adhere to Joint Commission (JC) guidelines, Conditions of Participation, and all applicable federal and state regulations.
  • Perform additional duties as assigned and needed.
Education
  • High School completion or GED (Minimum Level of Education Required).
Licensure / Certification
  • Medical Terminology course completed or currently enrolled.
Experience
  • 1–3 years of job‑related experience (Minimum Level of Experience Required).
  • Prior job title or occupational experience:
    Medical Office.
  • Preferred:
    Experience in Health Information or related medical field, and knowledge of Microsoft Word and Excel.
Knowledge / Skills
  • Ability to read and write to review and maintain patient medical record charts, perform basic math calculations, and operate standard office equipment.
  • Can work independently in a medical office setting.
  • Problem‑solving and basic analytical skills.
  • Excellent organizational skills.
  • Type accurately at acceptable speeds.
  • Strong verbal and written communication skills.
  • Interpersonal skills to effectively communicate with patients, physicians, and other personnel via telephone or direct contact to obtain and supply routine factual medical record information on a daily basis.
  • Basic analytical skills to determine work priorities, assist in problem resolution, and recommend workflow improvements.
