Mental Health Case Manager

Full Time - Mental Health Case Manager.

Mahajan Therapeutics is looking for a hard‑working and conscientious Case Manager to perform various administrative tasks with a keen eye for detail while working under the supervision of the Case Manager Supervisor and Clinical Director. The primary job function is to enhance the overall treatment process by assisting therapists in meeting the needs of clients. Performs other duties as assigned.

• Act as a liaison between therapist, the referring agency, and family of the client. Arrange family visits, coordinate transportation, and ensure clients’ needs are met.
• Ensure communication between family, referral source and Mahajan Therapeutics is agreeable and ongoing.
• Attend court hearings as deemed appropriate.
• Ensure documentation occurs according to policy.
• Attend treatment staffing with a prepared report that accurately represents client’s response to treatment.
• Facilitate individual and group CSPT as assigned.
• Function on agency quality assurance or other committees as assigned.
• Assist with in‑service training as directed by Clinical Director or CM Supervisor.
• Attend supervisory sessions with prepared agenda.
• Perform ongoing assessment of client needs.
• Assist in achieving personal independence in managing basic needs as identified by the client and/or parent or guardian.
• Facilitate further development of daily living skills if identified by the client and/or parent or guardian.
• Coordinate the ISP, including:
  • Services identified in the ISP.
  • Assistance with accessing natural support systems in the community.
  • Linkages to formal community services and systems.
• Monitor systems.
• Coordinate and assist in crisis management and stabilization.
• Advocate and outreach.
• When appropriate, provide education and training specific to the individual needs, abilities, and readiness to learn.
• Deliver mental health interventions that address symptoms, behaviors, thought processes, etc., to assist a client in eliminating barriers to seeking or maintaining education and employment.
• Engage in activities that increase the client’s capacity to positively impact the client’s environment.
• Have adequate knowledge and ability to navigate EHR (Electronic Health Record) system.
• Assume additional duties as assigned by Clinical Director or CM Supervisor.

• Bachelor’s degree in Social Work, a related field, or minimum two years of college credit plus five years of work experience in the field of Human Services or QMHS training adequate to perform job duties according to Ohio Law.
• Experience working with adolescents, teens, and adults, emotionally disturbed population, mental health experience preferred.
• Demonstrates a pleasant and professional manner with other employees, physicians, providers, patients, and patients’ families to promote a team effort through cooperation and respect for fellow employees.
