At Cherish Hospice, your role as RN Case Manager will orchestrate compassionate care, guiding families through the most vulnerable moments while ensuring the quality of life for patients. This full‑time opportunity is based in the Dayton area, traveling to homes, assisted living facilities, long‑term care centers, and hospitals.

• Lead and coordinate a multidisciplinary team of LPNs, aides, social workers, and chaplains to create personalized care plans.
• Advocate for each patient’s unique needs, wishes, and preferences while supporting families.
• Provide counseling and emotional support, building meaningful connections with patients and loved ones.
• Develop and implement care plans that prioritize comfort and quality of life.
• Document patient progress and care outcomes.
