Hospice Nurse After Hours

Salary: $80-89k/yr BOE

Schedule: Full-time | Sat/Sun/Mon 11 AM- 11 PM

Location: Dayton, Ohio (surrounding area)

• Licensed RN in the state of practice
• BLS Certified
• A minimum of 1 year of Clinical experience
• Valid Driver's License and Auto Insurance Coverage
• Reliable transportation:
  Willing to travel
• On-call/ triage experience preferred

• Provide on-call services after hours, and complete skilled visits if needed.
• Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
• Triage calls, coordinates services, and provides frequent updates on plan of care and any changes in condition.
• Assist reinforcement of patient and family education regarding end-of-life care.
• Participate in IDG and case conferences when needed.
• Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
• Respond to on-call or medical emergencies (nights and weekends).
