Crossroads Hospice is seeking a compassionate Nurse Practitioner to deliver high-quality palliative care across patient homes in the Dayton area. You will partner with families, assess symptoms, and coordinate end-of-life care with clinicians.
Travel between patient visits, document care plans, and ensure regulatory compliance are key aspects of the role. The ideal candidate has 2+ years of NP experience in acute care, an MS in Nursing, an OH NP license, and active DEA/NPI numbers.
