LPN/LVN role at Conviva Senior Primary Care
, located in Daytona Beach, FL (731 N. Clyde Morris Boulevard). The position is full‑time, Monday through Friday, with 40 hours per week.

The LVN assesses patient health problems and needs, takes vital signs, administers medication, develops and implements nursing care plans, and maintains medical records. The role may advise patients on health maintenance and disease prevention or provide case management. The LVN understands professional concepts, regulations, and operating standards within their work area, identifies work expectations and quality standards, and works with minimal direction.

• Current unrestricted licensure as an LPN/LVN in the state of Florida
• At least 2 years of experience working as an LPN/LVN

• Bilingual English/Spanish with ability to read, write and speak both languages
• Phlebotomy experience
• Experience with Electronic Medical Records (Athena)
• Experience with HEDIS
• Knowledge of Florida primary care medicine requirements
• Working knowledge of clinical operations
• Excellent customer service
• Strong clinical background
• Strong sense of engagement to be a team player
• Experience in a fast‑paced, high‑volume environment
• Passion for contributing to an organization focused on continuously improving consumer experiences

This role is patient‑facing and part of Humana’s Tuberculosis (TB) screening program. Candidates must be screened for TB if selected.

Pay Range: $45,400 – $61,300 per year (based on skills, experience, and geographic location)

This job is eligible for a bonus incentive plan based on company and/or individual performance.
