Case Manager - Autism Services

Pay rate will be between $18.00 to $22.00 per hour (and based on experience in that range)

Full-time position working 40 hours per week

Easterseals Florida is hiring a compassionate and organized Case Manager to support families navigating services through our Autism Center. This role is perfect for someone who thrives in a family-centered, collaborative environment and is passionate about helping children and caregivers access the resources they need.

• Welcome families, build rapport, and serve as their primary point of contact throughout the assessment and service process
• Guide families through autism screenings and assessments, providing education and support based on results
• Act as client advocate regarding intervention and treatment options
• Manage a client caseload, identify goals and develop individualized service plans
• Coordinate referrals to Easterseals programs and community resources
• Collaborate closely with physicians, therapists, internal teams, and community partners to ensure quality, timely, person-centered care
• Maintain accurate documentation in the electronic medical record and assist with assigned data tracking and reporting
• Stay informed on current research, services, and community resources to provide up-to-date information

• Bachelor’s degree in Social Work, Psychology, or a related human services field (required)
• At least 1 year of social work or human services experience (required)
• Knowledge of Autism Spectrum Disorder and family support needs (strongly preferred)
• Ability to collaborate with multidisciplinary teams and communicate effectively with families
• Strong organizational skills, initiative, and flexibility
• Proficiency in EMR systems and Microsoft Office
• Professionalism, integrity, tact, and good judgment
• Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)
