The Licensed Practical Nurse (LPN) plays a vital role in delivering compassionate, patient-centered care as part of Unity's dedicated healthcare team. Under the guidance of a Registered Nurse, the LPN provides direct patient care, supports the development and execution of care plans, and performs delegated nursing tasks to ensure the highest quality of service.
Schedule: 32-36 hours per week, working either 11:00 AM – 11:30 PM at the Meng Residence or 3:00 PM - 11:00 PM in the community, including every 3rd weekend rotation, and 2-3 holidays per year. This position is primarily based at the Meng Residence and may provide support in the community as needed, assisting patients in their homes or at partner facilities.
KeyJob Duties
- Provide basic nursing care
- Record nursing care given and report changes in a patient’s condition to the appropriate team member
- Assist with the development and revision of a nursing care plan
- Reinforce teaching provided by a registered nurse and provide basic health care instruction
- Perform delegated nursing acts under the direct supervision of a registered nurse
- Monitor IV fluids on controlled infusion pumps as directed by a registered nurse
- Maintain competency in required areas
- Comply with applicable laws and Unity policies and procedures
- Complete mandatory training requirements
- Document activity consistently and concisely according to department protocols
- Maintain confidentiality and professional boundaries
- Perform other job-related duties as assigned
- Maintain regular and reliable attendance
- Participate in seven days-a-week coverage rotation
- Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin
- Valid WI Driver’s License, reliable transportation, and proof of current automobile insurance
