Role overview
The Float Registered Nurse rotates duties between the community, the Meng Residence, and the Triage department depending on staffing needs. This includes completing skilled nursing assessments within the community or at the Meng Residence and providing safe and effective telephone advice, care instructions, and directions to the appropriate level of care. The nurse works collaboratively with the primary care teams, interdisciplinary team, and schedule coordinators to achieve high-quality patient standards.
During periods with a lighter caseload, you will support the team by assisting with patient visits and transitions and may serve as the Primary Registered Nurse as needed. This role is in the Green Bay territory and may travel outside of the territory.
Schedule
32 hours per week, 4 days a week, 3:00 pm – 11:00 pm. Weekend rotation is currently every 8th weekend, but may vary with a minimum of every 3rd weekend. Weekend shifts include Friday 11:00 pm – 7:00 am and Saturday 7:00 pm – 7:00 am. There is a backup rotation 10–15 times per year, and up to two holidays per week.
Key Job Duties
- Provide skilled nursing assessment and appropriate intervention
- Update the plan of care as necessary and review care plans with patient and family
- Respond to urgent patient/family needs in collaboration with triage nursing staff
- Initiate crisis care as needed
- Report appropriate findings and significant changes to the primary team, attending physician, and primary caregiver
- Provide education to patient/family on disease progression, symptom management, medications, and other care needs
- Administer and titrate medications according to orders
- Manage individual patient medications and contingency medications
- Record all orders in the Electronic Medical Record and initiate follow up as needed
- Prioritize daily work and adjust to the needs of patients, Unity, and other staff
- Communicate with interdisciplinary team members
- Delegate nursing duties to LPNs and supervise care provided by CNAs
- Complete new patient admissions as needed
- Telephone triage and follow-up to patient care contacts
- Record and maintain complete and accurate records of patient and physician communications
- Fax/call new and refill prescriptions according to physician orders
- Coordinate volunteer deliveries
Qualifications
Minimum Qualifications & Certification/Licensure
- Associate degree in Nursing required; bachelor’s degree preferred
- RN license in the state of practice required
- Valid WI Driver’s License, reliable transportation, and proof of current automobile insurance
Knowledge, Skills, and Abilities
- Computer proficiency (MS Office – Word, Excel, Outlook)
- Highly motivated and organized with strong attention to detail
- Effective verbal and written communication
- Ability to maintain confidentiality and exercise sound judgment
- Ability to stay up to date with policies, practices, and regulations
- Ability to prioritize and manage multiple tasks under deadline pressure
- Compliance with all applicable laws, Unity policies, and mandatory training requirements
- Ability to adapt to frequently changing demands
