Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations in Grand Rapids, MI. Visits are ~45 minutes, with a focus on bridging gaps in care using an iPad workflow.
You will travel locally within licensed states, conduct visits, and perform diagnostic/ preventive services as indicated, while not prescribing or altering treatment unless required by the plan.
