Licensed Practical Nurse Private duty

We are looking for a compassionate LPN to join our growing team! The right candidate has a can-do attitude, excellent interpersonal skills, and a current state LPN license. In this role, you will be providing medical support to our clients while ensuring a safe environment for them.

Dayshift 10am to 8pm Monday to Friday

Responsibilities:

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Clinical Supervisor.
• Uses equipment and supplies effectively and efficiently.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program
• Current state license as an LPN
• Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
• Proof of current CPR certification
• Excellent interpersonal skills
• Valid driver’s license and reliable transportation
• Ability to work a flexible schedule and travel locally
