Job Description

• Provide skilled nursing care in accordance with established standards and facility policies.
• Administer medications and treatments as prescribed, ensuring timely and accurate documentation.
• Monitor patient conditions, including vital signs, reporting significant changes to supervising RN or provider.
• Assist with patient assessments and contribute to care planning.
• Perform wound care, dressing changes, and other clinical procedures as required.
• Support residents with ADLs and ensure a safe, comfortable environment.
• Maintain accurate and organized medical records within regulatory guidelines.
• Respond effectively to emergencies and follow established protocols.
• Collaborate with interdisciplinary staff to ensure continuity of patient care.
• Provide compassionate and professional support to patients and their families.
