Community Residential Support Staff – Full Time
Base pay range: $16.36/hr.
Duties and Responsibilities- Support individuals in personalized activities of daily living and social skill development, including assistance in community, work, and home settings to help individuals learn, practice, and master valued social roles.
- Ensure the health, safety, and well‑being of individuals by following instructions from primary care physicians, nursing staff, and other relevant professionals.
- Promptly report changes in an individual’s condition or unusual occurrences to supervisors or management; report significant changes immediately.
- Report environmental risks, hazards, or barriers to supervisors or management in a timely manner.
- Safeguard individuals’ possessions, ensuring money and belongings are securely maintained.
- Assist individuals in maintaining order and cleanliness in their environment(s).
- Contribute to planning and implementation strategies of individualized support plans and behavior support plans.
- Implement Individual Support Plans (ISP) and Behavior Support Plans (BSP) as written, at the specified frequency.
- Demonstrate accurate data collection procedures and consistent use of support strategies that promote success.
- Document services provided, including individualized service goals, behavior support tracking, required notes, and all other required documentation.
- Support individuals in managing behavioral challenges and resolving minor problems.
- Complete incident reports using the online system, ensuring all required information is included.
- Transport individuals to medical appointments, community activities, or other events while ensuring safe transport and compliance with traffic laws.
- Maintain timely and effective communication regarding individuals’ needs and environment with appropriate personnel.
- Serve as a role model and mentor, demonstrating socially appropriate behaviors and coaching individuals using approved techniques.
- Support each individual's existing relationships and expand their network of friends and acquaintances.
- Ensure all individuals are supported to explore and express interests for community membership.
- Observe and maintain HIPAA confidentiality requirements concerning the affairs of the individuals served.
- Complete all scheduled and required training by announced deadlines, and actively participate in home staff meetings.
- Actively contribute to the agency’s continuous improvement efforts by sharing ideas and insights.
- Facilitate teamwork between all team members through open communication and a positive, constructive attitude.
- Interact professionally with colleagues, families, guardians, support coordinators, and internal/external monitors.
- Complete other duties as assigned.
- Assist with activities of daily living, including cooking, cleaning, laundry, dishes, household maintenance, community involvement, personal hygiene, leisure activities, communication, personal safety, health maintenance, medication administration/management, and social/behavioral development.
- Document services, including individualized service goals, behavior support tracking, required notes, communication logs, medication administration records, and all other required documentation.
- Maintain proper communication regarding individuals’ needs and home environment with appropriate personnel, reviewing the communication log at shift start and completing it at shift end.
- Plan, prepare, and grocery shop for meals, following special diets and nutritional instructions.
- Follow Supervised Self‑Administration of Medication (SSAM) and Proxy Caregiver procedures without errors, including reading and following medical and nursing instructions.
- High school diploma or GED, plus any combination of training and experience that enables the applicant to acquire necessary knowledge, skills, and abilities.
- Possess a valid Georgia driver’s license and acceptable Motor Vehicle Report (MVR).
- Basic computer skills and the ability to advocate for individuals, interpret written materials, prepare incident reports, and resolve conflicts.
