Job Code: 35080 – Salary Grade: 17 – Salary Range: $52,266 - $84,148 – FLSA Status:
Exempt.

Essential Functions
  • Develops, implements, and monitors comprehensive plan of care for clients based on assessments, client expectations, and availability of services; initiates/coordinates services and maintains contact with service providers; collaborates with client, family, and informal supports to implement plan; reviews and updates care plans; evaluates service provisions; monitors progress; and adheres to federal, state and agency policies regarding service eligibility and monitoring.
  • Conducts individual counseling sessions and facilitates group counseling sessions utilizing evidence-based curriculum. May work directly with mentally ill and/or developmentally disabled persons involved with the criminal justice system and provide counseling/therapy to participants requiring complex intervention, including those exhibiting homicidal, suicidal, life‑threatening behaviors; and facilitates counseling groups and provides assistance to participants outside normal business hours when needed.
  • Maintains client case files in departmental database; documents individual counseling sessions, group sessions, and other services provided to notate client progress; inputs, updates, and changes information as needed, develops and maintains updates individualized treatment plans, and audits clinical treatment files.
  • Coordinates with treatment court staff to ensure cohesion and collaboration between the team; schedules group coverage and assignments; provides treatment recommendations to staff and court representatives; and performs drug screens on participants within program when necessary.
  • Attends status review meetings and court proceedings involving client cases.
  • Maintains a comprehensive, current knowledge of applicable policies, procedures, regulations, codes, and other regulatory standards related to aging services; maintains awareness of available community services and resources; attends workshops, seminars, and training sessions as appropriate.
Minimum Qualifications
  • Bachelor’s degree in Social Work, Psychology, Public Health, Criminal Justice, or a related field.
  • Five years experience working within the criminal justice system, addiction counseling, or related field.
  • Experience working within an accountability court model, and experience with delivering evidence-based practices preferred.
  • Specific License or Certification

    Required:

    LMSW, LCSW, LAPC, LPC, CADC II, or CAC II. Must possess and maintain a valid Georgia driver’s license.
