PHYSICAL THERAPIST IV - IP at Memorial Health
Compensation: USD $44.47/hr. up to USD $68.93/hr sign‑on Bonus up to $20,000.00
OverviewThe Physical Therapist IV is a community resource for a specific diagnostic group. Demonstrates advanced competency through specialized certification credentials and physician validation. Responsible for examinations, evaluations, diagnosis, interventions, and prognosis in accordance with hospital policies. Leads performance improvement, program development, and marketing. Facilitates educational opportunities and implements evidence‑based practice.
ScheduleFull time, Days. $20,000 Sign‑on Bonus.
Qualifications- Minimum 8 years of experience.
- P.T. license in the state of Illinois.
- CPR certification preferred.
- Valid Illinois driver’s license with acceptable driver status per MHS Fleet Safety Policy.
- External candidates require management approval.
- Achieve and maintain >31 points on the Clinical Ladder Grid.
- Basic computer skills.
- Effective communication with patients, families, peers, and team members.
- Physically lift, support, and move patients 50–100 lbs.
- Perform patient assessments and assign a physical therapy treatment diagnosis.
- Establish plan of care with objective, measurable goals.
- Provide interventions supported by evidence‑based practice.
- Embed Safety, Courtesy, Quality, and Efficiency standards.
- Assess effectiveness of treatment and modify plans to achieve goals.
- Plan for discharge and communicate with all stakeholders.
- Document care to meet quality standards and ethical billing.
- Organize workload and manage time efficiently.
- Develop collaborative relationships with physicians.
- Triage patient care during high volumes.
- Serve as a role model for difficult or stressful situations.
- Develop annual plans for clinical expertise.
- Share expertise through evidence‑based practice, research, and conferences.
- Serve as a resource within professional associations.
- Perform related duties as required.
