Salary: USD $37.43/Hr – Max USD $58.02/Hr.
OverviewThe Physical Therapist II is responsible and accountable for providing examinations, evaluations, therapy diagnosis, interventions, and prognosis in accordance with established hospital and departmental policies and procedures. The Therapist II will share their knowledge base through serving as a clinical instructor for students, mentoring new staff, and participating in community education activities. The Therapist II will participate in performance improvement and program development activities.
Qualifications- Bachelor’s degree in Physical Therapy or entry-level equivalent is required.
- P.T. license in the State of Illinois required.
- CPR Certification preferred.
- If applicable, must possess a valid Illinois driver’s license and be deemed an acceptable driver per MHS Fleet Safety Policy (5 year MVR required).
- Current CPR card.
- Basic computer skills required.
- Ability to communicate effectively with patients, families, peers, and team members.
- Physically able to lift, support, move patients 50–100 lbs.
- Universal Precautions Category I.
- Achieves & maintains 11-20 points on the Clinical Ladder Grid.
- 2 years of experience required.
- Ability to organize workload and manage time effectively.
- Ability to handle difficult and/or stressful situations appropriately.
- Demonstrates teamwork by routinely offering to help others or assisting others whenever needed.
- Maintains positive working relationship with patients and referral base.
- Perform patient assessments with patients in a specific service area.
- Assign a physical therapy treatment diagnosis.
- Establish a plan of care with objective, measurable goals.
- Provide interventions supported by evidence-based practice in accordance with the established plan of care.
- Assess the effectiveness of treatment and modify treatment to achieve goals.
- Plan for discharge.
- Communicate with patient, family, caregivers, and members of the team to promote maximum benefit of care.
- Provide patient care documentation that meets quality standards and supports ethical billing practice.
- Demonstrate ability to organize workload and manage time effectively.
- Maintain positive working relationship with patients and referral base.
- Demonstrate teamwork by routinely offering to help others or assisting others whenever needed.
- Demonstrate ability to handle difficult and/or stressful situations appropriately.
- Share knowledge through service as a clinical instructor and mentoring new staff.
- Share knowledge with department and community through participation with educational offerings.
- Perform other related work as required or requested.
