Licensure / Certification / Registry
  • O.T. license in the State of Illinois, or license pending with an application for licensure on file with IDPFR and a letter of “Authorization to Work” from the State of Illinois (maximum practice time 6 months).
  • CPR Certification preferred.
  • If applicable, valid Illinois driver’s license; must meet MHS Fleet Safety Policy (5‑year MVR required).
Other Knowledge, Skills & Abilities
  • Achieves & maintains 0‑10 points on the Clinical Ladder Grid.
  • Basic computer skills required.
  • Ability to clearly and effectively communicate with patients, families, peers and team members for assessment, intervention, and professional development.
  • Physically able to lift, support, move patients 50–100 lbs.
  • Knowledge of Universal Precautions Category I.
Responsibilities
  • Perform patient assessments in specific service area.
  • Assign an occupational therapy treatment diagnosis.
  • Establish plan of care with objective, measurable goals.
  • Provide interventions supported by evidence-based practice in accordance with established plan of care.
  • Assess effectiveness of treatment and modify treatment to achieve goals.
  • Plan for discharge.
  • Communicate with patient, family, caregivers and team to promote maximum benefit of care.
  • Provide patient care documentation that meets quality standards and supports ethical billing practice.
  • Demonstrate ability to organize workload and manage time effectively.
  • Maintain positive working relationship with patients and referral base.
  • Demonstrate teamwork by routinely offering help or assisting others when needed.
  • Handle difficult or stressful situations appropriately.
  • Perform other related work as required or requested.
Physical Demands
  • Walk, stand, bend, and reach.
  • Lift, push, and pull weight of at least 1–5 pounds.
Mental Demands
  • Work under stress and adapt to changing conditions.
