The Care Navigator coordinates the admitting and transferring of residents for Adams Woodcrest, Adams Heritage, and Adams Memorial Swing Bed facilities. Strives to provide an environment conducive to resident safety and comfort. Demonstrates competence in general nursing skills and is conversant in Medicare, Medicaid, and State/Federal guidelines for long-term care facilities. Maintains a working relationship acting as liaison with discharge planners, insurance carriers, physicians, and families.
Maintains compliance with initial prior authorization for resident care. Must have a willingness to have flexible hours to meet the needs of the hospital and nursing facility. Develops a department budget in coordination with the executive director. Performs other duties as assigned by Director of Senior Living.
Eligible for medical coverage on your 1st day of employment; all other benefits will be effective the 1st of the month following hire date.
Requirements- 32 hours per week – First Shift
- 8:00 AM - 4:30 PM
- On-call Every Other Weekend
- Current LPN License in the State of Indiana
- CPR Certification
