Pay RangeBase pay: $85,000.00 – $98,000.00 per year
Job Responsibilities- Complete an initial assessment of patient and family to determine hospice care needs
- Regularly re‑evaluate patients’ nursing needs
- Collaborate with the healthcare team, patient and family to coordinate continuous, well‑coordinated care
- Counsel the patient and family in meeting end‑of‑life needs
- Initiate appropriate preventative and palliative nursing procedures
- Communicate with physicians regarding patient needs and report any changes in the patient’s condition
- Supervise Hospice Aides
- At least 1 year of nursing experience in hospice, home health, or critical care (ICU/Intensive Care)
- Associate degree in nursing
- Active and clean Illinois Registered Nurse license
- Hospice experience preferred
- Valid driver’s license and reliable transportation
