Deeside Medical Centre in Shotton is recruiting a practice nurse to work within a small general practice. The work centres on chronic disease reviews, immunisations, cervical screening and anticoagulation monitoring, alongside clinical documentation and governance. The post also covers health promotion, disease prevention and the care of acute and long-term conditions. Current NMC registration and general practice experience are essential.
What the job involves- Managing chronic diseases and carrying out chronic disease reviews
- Delivering immunisations and vaccinations, including immunisation programmes
- Carrying out cervical screening
- Monitoring patients on anticoagulation
- Maintaining documentation and clinical governance
- Supporting the delivery of essential primary care services
- Promoting health and preventing disease
- Managing acute and long-term conditions
- Delivering safe, effective and compassionate patient care in line with current clinical guidance and practice policies.
Registration & licences
- Current NMC registration
- Experience in general practice
- Experience in long-term condition management
- Training or experience in immunisations
- Training or experience in cervical screening
- Training or experience in chronic disease reviews
- Following infection prevention and control policies
- Complying with safeguarding procedures and reporting concerns appropriately
- Knowledge of relevant primary care clinical systems and national guidelines
- Participation in clinical audits, quality improvement and incident reporting
