Nurse Practitioner – Long-Term Care / Skilled Nursing Facilities

We are seeking a dedicated and compassionate Nurse Practitioner to provide high-quality primary and preventative care to residents in Skilled Nursing Facilities (SNFs). This role is ideal for an experienced clinician who thrives in a collaborative environment and is committed to improving outcomes for patients with complex, chronic, and advanced illnesses.

Key Responsibilities

Clinical Care

• Provide comprehensive primary and preventative care to long-term care residents residing in Skilled Nursing Facilities.
• Assess, diagnose, and manage residents experiencing an acute change in condition.
• Conduct advanced care planning and provide chronic care management tailored to patient needs.
• Prescribe medications, including controlled substances, in accordance with Ohio state law and scope of practice.
• Order, review, interpret, and follow up on diagnostic tests, treatments, and laboratory results.
• Communicate regularly with nursing home staff to discuss changes in condition, treatment updates, and care plans.
• Collaborate closely with the primary care physician and the interdisciplinary care team to coordinate high-quality patient care.
• Participate in care discussions to ensure patient needs are managed effectively and comprehensively.

Schedule / Shift

• Monday–Friday, 8:00 AM – 5:00 PM EST
• 4-day workweek option available (specific days to be coordinated with manager)

Qualifications (Must-Haves)

• Current, unrestricted Nurse Practitioner license in the State of Ohio.
• Board Certification (Adult, Family, or Geriatric specialties).
• Active DEA license.
• Experience working in a long-term care, skilled nursing, or similar setting.
• Proficiency with Electronic Medical Records (EMR).
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Strong ability to work collaboratively within an interdisciplinary care team.
• Psych experience is a plus
