Overview

As a Home Health PT at Inspired Homecare, you will directly impact patients by delivering high-quality care in the home setting. Your day-to-day responsibilities include assessments, creating personalized treatment plans, and coordinating care with a diverse team of healthcare professionals to ensure seamless delivery of services. You will monitor patient progress, adjust treatment plans as needed, and provide ongoing support to help patients achieve their goals.

Flexible scheduling is available to accommodate your needs (weekdays, weekends, or a combination).

Responsibilities
  • Conduct comprehensive physical therapy assessments in patients' homes.
  • Create and implement personalized treatment plans.
  • Deliver high-quality care and monitor patient progress.
  • Adjust treatment plans as needed based on patient response.
  • Collaborate with a diverse team of healthcare professionals to ensure coordinated care.
  • Provide ongoing support to help patients achieve their goals.
Qualifications
  • A degree in Physical Therapy from an accredited university/program.
  • A current, unrestricted state license as a Physical Therapist in the state of practice.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
  • Strong organizational and communication skills.
  • Experience in Home Health Therapy and completion of OASIS documentation is preferred.
