Base pay range: $70,500.00/year – $96,900.00/year. Additional pay per visit/unit: $45.00–$63.00.
Responsibilities- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
- Collaborate with physicians to develop initial plans of treatment based on orders and assessments; revise plans in consultation with physicians.
- Coordinate with multidisciplinary staff (physical therapists, occupational therapists, home health aides, external providers).
- Report patient care/condition/progress to physician and Clinical Manager continuously.
- Implement care plans with patient/family to achieve optimal outcomes.
- Supervise and oversee home health aides and personal care workers, providing written instructions and monitoring documentation.
- Discharge patients after consultation with physician and Clinical Manager.
- Prepare comprehensive medical documentation for all patients, including conferences, medication changes, re-certifications, progress updates, and care plan changes.
- Diploma, Associates or Bachelor’s degree in Nursing.
- Minimum of one year of nursing experience (clinical surgical, ICU, ER, acute).
- Home Health experience preferred.
- Current, unrestricted Registered Nurse license.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
