Base pay range: $70,500.00/year – $96,900.00/year. Additional pay per visit/unit: $45.00–$63.00.

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
• Collaborate with physicians to develop initial plans of treatment based on orders and assessments; revise plans in consultation with physicians.
• Coordinate with multidisciplinary staff (physical therapists, occupational therapists, home health aides, external providers).
• Report patient care/condition/progress to physician and Clinical Manager continuously.
• Implement care plans with patient/family to achieve optimal outcomes.
• Supervise and oversee home health aides and personal care workers, providing written instructions and monitoring documentation.
• Discharge patients after consultation with physician and Clinical Manager.
• Prepare comprehensive medical documentation for all patients, including conferences, medication changes, re-certifications, progress updates, and care plan changes.

• Diploma, Associates or Bachelor’s degree in Nursing.
• Minimum of one year of nursing experience (clinical surgical, ICU, ER, acute).
• Home Health experience preferred.
• Current, unrestricted Registered Nurse license.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.
