General Summary

The pharmacist is a healthcare professional responsible for safe, effective, timely medication distribution and administration.

Responsibilities
  • Medication/Solution Order Review and Processing — Review prescriber’s orders for appropriateness relative to the patient’s condition, height, weight, allergies and age. Ensure compliance with legal requirements, institutional policies, and drug use restrictions. Check for duplicate therapy, drug-drug interactions, dosage range, IV incompatibility, and food-drug interactions to facilitate accurate and timely medication distribution and administration.
  • Drug Information Resource — Provide pertinent drug information to medical and ancillary hospital staff. Research dosages, indications, interactions, compatibility, and administration as requested; respond timely and professionally.
  • Departmental Clinical Program Participation — Complete pharmacy consults timely and accurately; participate in multi-disciplinary rounds.
  • Technician and Ancillary Staff Supervision — Review medications prepared by technicians for accuracy and timeliness; monitor adherence to policies and procedures. Validate intravenous admixtures, parenteral nutrition, chemotherapy and sterile products. Ensure dispense prep meets quality standards.
  • Departmental Performance Improvement Participation — Participate in routine quality control and improvement activities; monitor ADC reports, narcotic inspections, and support safety and improvement initiatives.
  • Controlled Substance Accountability — Manage chain of custody for controlled substances, including dispensing, storage, hand-off, and investigation/clearing of open transactions.
  • Adverse Drug Reporting — Identify, report and document potential adverse drug reactions.
  • All other duties as assigned within scope of responsibilities.
Required Education, Credential(s) and Experience
  • Education:

    Bachelor Degree
  • Credential(s):
    Licensed Pharmacist
  • Experience:

    Preferred: 1 year of hospital experience
Preferred Education, Credential(s) and Experience
  • Education:

    Doctorate Degree Pharmacy
