Provide patient specific regimens to avoid SUD in patients starting opioids and in patients with a known or suspected history of SUD; this includes patient and caregiver education.
  • 2. Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data. Utilize laboratory screening for opioids and illicit substances to help formulate a plan to treat patients with substance use disorder.
  • 3. Initiate, titrate, and manage evidence-based pharmacotherapy for SUD including follow-up and monitoring for withdrawal symptoms and adverse effects. Use shared decision-making with the healthcare team to promote collaborative, evidence-based, patient-centered care with treatment goals.
