** Delaware Hospice is actively seeking a full-time Dementia Care Navigator.
** The
** Dementia Care Navigator
** is a key member of the Delaware Hospice Dementia Care GUIDE Program, serving as the primary non-clinical point of contact for individuals living with dementia and their caregivers. The role provides relationship-based caregiver support, care coordination, and navigation services using standardized workflows and technology-enabled tools, in alignment with the CMS Guiding an Improved Dementia Experience (GUIDE) Model.
The Care Navigator works collaboratively with the interdisciplinary team members to support high-quality, consistent dementia care delivery, caregiver engagement, and program scalability. This is an exempt position.

Qualifications
* Minimum of two (2) years of experience in care coordination, patient navigation, case management, or healthcare‐related service delivery.
* Experience working with older adults, individuals with dementia, and/or family caregivers preferred.
* Strong interpersonal, communication, documentation, and organizational skills.
* Comfort using care management systems, electronic health records, and other technology‐based tools.
* Dementia Care certification/training required (provided through Delaware Hospice).
* Valid driver’s license required.
* Ability to work remotely in a hybrid model with periodic in‐person or community‐based engagement as required.
