(Responsibilities vary by client)

• Follow an individualized plan of care

• Provide companionship and emotional support

• Light housekeeping and meal preparation

• Transportation to appointments and errands

• Personal care (bathing, toileting)

• Communicate professionally with families and your care team

• Other duties as assigned
