The Hospice Aide provides personal care and assistance to hospice patients under the direction and supervision of the RN/Case Manager and in accordance with the patient's individualized Hospice Aide Plan of Care.

Following the assigned care plan and completing accurate, timely documentation are essential responsibilities of this position. Care must be provided exactly as directed, and all required documentation must clearly reflect the services provided, observations made, and any changes or concerns identified during the visit.

Being of service to others is more than a career; it is a calling. Tranquility Hospice is committed to supporting our employees while creating a positive, team-oriented work environment centered on compassionate end-of-life care.

Position Responsibilities Following the Assigned Care Plan
  • Follow the patient's assigned Hospice Aide Plan of Care and all patient‑specific instructions provided by the supervising RN/Case Manager.
  • Perform only the care and services authorized within the assigned Plan of Care, applicable regulations, training, and organizational policy.
  • Review assigned care instructions prior to providing services and ensure all required tasks are completed as directed.
  • Immediately communicate any barriers to completing the assigned care plan to the RN/Case Manager.
  • Do not independently add, remove, or change patient‑care tasks without direction from the appropriate supervising nurse.
  • Maintain consistency with the patient's current Plan of Care throughout each visit.
Personal Care
  • Provide personal care to patients in accordance with the individualized Hospice Aide Plan of Care and instructions provided by the supervising RN.
  • Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, oral care, feeding, and other authorized personal‑care needs.
  • Assist with ambulation, positioning, transfers, mobility, and passive range‑of‑motion activities as directed by the Plan of Care.
  • Assist with meal preparation and other appropriate activities necessary to support the patient's care and comfort.
  • Provide care in a manner that promotes patient dignity, privacy, comfort, safety, and independence whenever possible.
Documentation
  • Complete all required documentation accurately and promptly following each patient visit.
  • Ensure documentation clearly reflects the care and services actually provided in accordance with the assigned Hospice Aide Plan of Care.
  • Document personal care completed, patient observations, patient response to care, changes in condition, refusals of care, incomplete tasks, concerns, and other required clinical information.
  • Ensure documentation is complete, accurate, relevant, and submitted within required organizational time frames.
  • Never document care or services that were not actually provided.
  • Promptly notify the RN/Case Manager when an assigned task cannot be completed, a patient refuses care, or the patient's condition or needs have changed.
  • Understand that accurate documentation is a vital component of patient safety, continuity of care, regulatory compliance, and communication with the interdisciplinary team.
