Tranquility Hospice is seeking a dedicated LPN to join our Delaware, OH area team. You will provide hospice nursing care, collaborate with the RN/Case Manager, and support patients and families with compassionate, evidence-based care.
The role includes administering medications, documenting in the EHR, and coordinating with the interdisciplinary team to meet each patient’s plan of care. A valid OH LPN license and hospice experience are preferred.
