Working under the supervision of the Police or Fire Chief or their designee, the Service Coordinator will work with individuals in the Delaware community who encounter Police or Fire personnel through the course of requesting emergency services. This position will assist in evaluating the individuals needs for mental health, physical or daily living services and assist them in connecting with the appropriate community resources and organizations who can further assist in meeting those needs.

Position requires regular and reliable attendance.

Essential Functions: For purposes of 42 USC 12101

  • Accepts and processes referrals of individuals and families from the CIT (Crisis Intervention Training) and RES (Rapid Engagement Services) Data Sharing process of the Delaware City Police Departments, as well as directly from law enforcement/fire/EMS personnel when appropriate.
  • Interacts with clients, makes referrals, and educates them on resources in the community.
  • Actively manages CIT Data Sharing reports, including reviewing referrals and screening for appropriate behavioral health and/or healthcare linkage.
  • Facilitates follow up to provider system when individual is not currently connected to services.
  • Engages referred individuals in the community and in their home environment to determine behavioral health and other needs when necessary.
  • Provides service coordination including screening, intake, education, advocacy and referral of individuals and families in crisis to service providers in the community.
  • Provides screening and initial intervention, determines behavioral health needs, and connects identified individuals to appropriate care when needed. Accurately assesses needs and links the individual to appropriate community resources.
  • Provides ongoing follow up and linkage services to referred individuals until actively engaged in services or immediate needs are met. Provides short to intermediate term case management when appropriate.
  • Documents referral activities and provides regular updates to facilitate the progress of cases.
  • Develops and maintains positive working relationships with community behavioral health and other healthcare providers, local law enforcement, and other community systems. Works cooperatively to ensure that individuals needs are met.
  • Provides follow up information to referent law enforcement, fire and EMS when permitted to facilitate timely and effective response.
  • Networks with community agencies and providers.
  • Initiates assistance to individuals in building informal support networks with other residents, family, and friends.
  • Attends staff meetings, supervision with immediate supervisor, training and other required meetings and events as scheduled.
  • Other duties as assigned.
