HJ Staffing is currently seeking an experienced and compassionate Case Manager to join the Substance Use Disorder Program for Offenders team on assignment with a government agency. This position plays a vital role in helping justice-involved individuals access services, track progress, and achieve successful outcomes.
Key ResponsibilitiesProvide intensive case management services to clients enrolled in the SC PORT program.
Assess client needs and develop individualized service plans focused on recovery and rehabilitation.
Maintain accurate and timely documentation in compliance with program policies and procedures.
Coordinate referrals to treatment providers, housing, employment, and other support services.
Monitor client progress, attend case staffing meetings, and adjust service plans as needed.
Prepare and deliver reports, performance data, and program updates to stakeholders.
Collaborate with justice system partners, behavioral health agencies, and internal departments.
Bachelor’s degree in Social Work, Psychology, Public Administration, Criminal Justice, or a related field.
At least 3 years of case management experience, preferably with justice-involved populations.
Strong understanding of substance use treatment and behavioral health services.
Excellent communication, documentation, and organizational skills.
Ability to work independently and manage a caseload in a high-paced, supportive environment.
Master’s degree in Social Work or related discipline.
Experience working with government-funded or court-related programs.
Bilingual (English/Spanish) is a plus.
