Kessler Perspektief zoekt een Trajectondersteuner voor een woon-/leeflocatie in Delft. Je zorgt voor een stabiele dagindeling, deelt medicatie en ondersteunt bij maaltijden en ADL, met aandacht voor veiligheid en welzijn van cliënten.
Je werkt volgens een onregelmatig rooster met dag-, avond-, weekend- en maximaal 3 nachtdiensten per maand. Je hebt minimaal MBO-4 Sociaal Werk en flexibiliteit. Een introductieprogramma en trainingen horen erbij.
