Ipse de Bruggen zoekt een Begeleider HBO voor onze locatie Paardenmarkt in Delft. Je coördineert dagelijks de gang van zaken, zorgt voor structuur in de zorg en begeleidt cliënten bij verzorging, eetmomenten en activiteiten, afgestemd op individuele behoeften.
Daarnaast coach en ondersteun je collega's zodat het team optimaal functioneert. Je signaleert tijdig issues en draagt bij aan verbeteringen, zodat bewoners zich veilig en gewaardeerd voelen en de woning elke dag tot zijn recht komt.
