Ipse de Bruggen zoekt een persoonlijk begeleider mbo4 voor een 24-uurs woonlocatie in Delft. Je ondersteunt cliënten met een lichte tot matige verstandelijke beperking bij het begrijpen van hun omgeving en het opbouwen van gezonde relaties, in nauwe samenwerking met familie en professionals.
Je vertaalt mogelijkheden naar een persoonlijk zorgplan, coördineert de zorg en bent het eerste aanspreekpunt voor cliënt en verwanten.
