The Physical Therapist is responsible for delivering medically prescribed, goal‑oriented therapy in accordance with the Physician's Plan of Care
, company policies, and professional standards of practice. This role collaborates closely with an interdisciplinary team to coordinate care and achieve optimal patient outcomes.
- Develop and implement individualized physical therapy plans of care
- Provide in-home visits, including evaluation and reassessment of patient needs
- Communicate effectively with physicians and interdisciplinary care team members
- Educate patients, families, and caregivers on treatment plans and exercises
- Participate in interdisciplinary meetings to coordinate and review care
- Ensure care plans are updated, reviewed, and documented accurately
- Complete electronic documentation timely and accurately in the EMR (Well Sky/Kinnser preferred)
- Adhere to agency policies, state and federal regulations, and ACHC standards
- Active Florida Physical Therapist (PT) license in good standing
- Specialized training and experience in Pelvic Floor Therapy
- Strong knowledge of physical therapy principles and methodologies
- Excellent written, verbal, and interpersonal communication skills
- Strong critical thinking, problem‑solving, and decision‑making abilities
- Ability to collaborate effectively within an interdisciplinary team
- Current CPR certification
- Level 2 background screening
- Recent negative TB test or chest X‑ray
