Director of Health Care Transformation

Transform primary care as the Director of Health Care Transformation in South Delta. This role requires a collaborative leader dedicated to enhancing community-based health initiatives.

As the Director, your primary focus will be the successful operationalization of the Ministry of Health's PCN initiative, working with stakeholders like local health authorities and First Nations. Your leadership will guide the implementation of the PCN Service Plan, utilizing your expertise in quality improvement and change management to foster a culture of engaged health care.

Key Responsibilities:

• Operationalize the PCN initiative within the region

• Lead transformation of primary care systems

• Cultivate relationships with health authorities and communities

• Promote principles of population health and health promotion

• Leverage project management skills for effective collaboration

Requirements:

• Minimum of 5 years in health sector leadership

• Experience in quality improvement and cultural safety

• Required degree in health-related field

• Preferred master’s degree

• Strong leadership and facilitation skills
