In this role, you will ensure continuous client engagement, develop tailored treatment strategies, and integrate community services into your practice. Key Responsibilities:
• Develop individualized treatment and support plans
• Facilitate client intake and assessment processes
• Research and connect clients with community services
• Provide client-focused advocacy and support
• Engage in quality improvement initiatives
Requirements:
• Bachelor’s degree in a relevant health discipline
• Experience in mental health/substance use services
• Registration eligibility with a professional body
• Valid Class V BC Driver's License
• Strong case management and counseling skills
