Fraser Health is looking for a Licensed Practical Nurse to support its mission within the Hospice team at Delta Hospital, located in Delta, B.C. You will perform assessments, develop care plans, and operate essential medical equipment, ensuring every patient feels respected. This position is vital for enhancing patient comfort and safety.

Key Responsibilities:

• Conduct thorough assessments of patient needs

• Promote patient involvement in their care process

• Participate in multidisciplinary care planning

• Document patient care accurately and thoroughly

• Maintain a hygienic environment for patient safety

Requirements:

• Completed practical nursing program or equivalent training

• Active registration with BCCNM

• Strong ability to communicate effectively

• Capacity to execute care plans and adaptations

• Physical fitness to perform duties of the role
