This unique position combines  virtual and in-person care , giving you the opportunity to work directly with clients in their homes and communities while collaborating with an interdisciplinary healthcare team to optimize medication therapy and support healthy aging t You'll Do   Provide comprehensive medication assessments and individualized pharmacotherapy recommendations
Collaborate with physicians, nurses, and other healthcare professionals to support Home Health clients
Deliver medication counselling and education to patients and caregivers
Lead medication safety and quality improvement initiatives
Participate in research, practice development, and clinical education
Mentor pharmacy students, residents, and new team members
Serve as a trusted pharmacy resource within the community
What You Bring   Current registration with the College of Pharmacists of BC
Completion of an accredited Hospital Pharmacy Residency Program
Location & Work Environment  This position supports the  Fraser Health Home Health Program  and is based out of the  Surrey-North Delta Home Health Office at Gateway Station Tower  Avenue, Surrey, BC) .
A vehicle is required for work-related travel. Mileage and applicable insurance costs are reimbursed in accordance with the Collective Agreement.

Detailed Overview  The Clinical Pharmacist, Community Pharmacy Programs,provides clinical pharmacy services for community dwelling clients including individualization of client medication therapy; client medication counselling; consultation with physicians, nurses and other health care providers to develop and discuss therapeutic plans and assistance in the development of drug therapy guidelines for individual disease conditions and provides academic detailing sessions for physicians to optimize prescribing practices.
Provides support to the Pharmacy & Therapeutics Committee by preparing written summaries and recommendations on drug usage; contributes items such as case studies and articles for submission to professional journals; acts as department resource person in the assigned area of responsibility; participates in medication use evaluations; provides instruction and/or supervision to pharmacy students and residents.
Responsibilities   Consults with physicians, nurses and other health care providers to develop and discuss therapeutic plans appropriate for each client based on medication appropriateness, client environment and assessment of clinical responses. Ensures appropriate documentation is maintained in the client health record in accordance with regulations, protocols and practice standards.
Individualizes client medication therapy by performing activities such as referencing standard literature/publications and client demographic information, reviewing client charts and laboratory test results, meeting with the client and health care team to assess clinical response of the client to medication therapy and modifying or developing therapeutic plans to ensure that medication therapy is consistent…
