The Clinical Social Worker position at Fraser Health requires candidates who possess a Bachelor’s degree in social work combined with two years of relevant experience, ideally in complex healthcare settings. You will conduct psychosocial assessments, develop care plans, and provide comprehensive therapeutic and crisis intervention services. This role emphasizes collaboration within a multidisciplinary team while ensuring culturally safe and client-centered care.

Key Responsibilities:

• Conduct in-depth psychosocial and social work assessments

• Provide therapeutic support and crisis intervention to clients

• Collaborate with the interdisciplinary team in care planning

• Maintain patient records and ensure documentation compliance

• Facilitate in-services and support groups for staff and community

Requirements:

• Full registration with the BC College of Social Workers

• Two years relevant experience in complex healthcare

• Valid B.C. Driver’s license and access to a vehicle

• Comprehensive knowledge of counseling models and health legislation

• Ability to work independently and as part of a diverse team
