The Admissions Registered Nurse (RN) is responsible for assessing and admitting patients into PACE. The Admissions RN performs clinical evaluations, ensures compliance with regulatory guidelines, coordinates with interdisciplinary and enrollment teams, to initiate the participant care plan to ensure a smooth transition into care. In addition, this nurse will support nursing needs in both the home/community or clinic as needed.
What is PACE?PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.
The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.
You Bring:- Graduated from an accredited school of nursing is required. Bachelor's degree in Nursing is preferred.
- Minimum of 1–2 years of nursing experience, preferably in admissions, emergency room, home health, hospice, or acute care.
- Current, valid Registered Nurse (RN) license in the state of Colorado, maintained throughout employment.
- Valid Colorado driver's license, reliable transportation, and proof of required automobile insurance maintained throughout employment.
- Must possess and maintain current CPR certification throughout employment.
- Ability to set priorities, manage competing demands, and consistently follow through.
- Strong interpersonal, verbal, and written communication skills.
- Ability to communicate clearly, effectively, and respectfully in a wide range of situations, including highly emotional or stressful interactions.
- Proficiency with electronic health record (EHR) systems and Microsoft Office applications.
