Step into a meaningful role as a Licensed Practical Nurse with Fraser Health, where you'll focus on Indigenous health care delivery. Work in community and hospital environments to improve access and provide culturally sensitive care.
In this LPN position, you will actively contribute to the well-being of Indigenous clients by assessing their health needs and navigating services available through Fraser Health.
Your role involves developing personalized care plans, advocating for clients, and promoting understanding of Indigenous health practices among healthcare teams. You will also support discharge planning and facilitate connections to community resources.
Key Responsibilities:
• Collaborate to create culturally relevant care plans
• Conduct initial and ongoing client assessments
• Offer health education and promote wellness strategies
• Communicate with agencies for seamless service integration
• Document client care comprehensively and accurately
Requirements:
• Active LPN registration with BCCNM
• Minimum one year of relevant nursing experience
• Familiarity with Indigenous health issues and protocols
• Access to a personal vehicle for work
• Excellent problem-solving and communication abilities
